Coronary Artery Angiography (Cardiac Catheterization)

This condition primarily affects the myocardium, which is the middle layer of the cardiac tissue. If there is a lock of oxygen, the myocardial cells will die, thus decreasing contractility, stroke volume and blood pressure. Specifically, when the left ventricle is damaged there may be an inability for the heart to pump properly to allow blood. Background Vascular closure devices following cardiac catherization through the femoral artery have been gaining significant traction since its use in the mids. Compared to the previous gold standard of manual compression, VCDs allow for shortened time to ambulation, decreased duration of hospitalization post procedure, and increased patient comfort.

The devices have also shown safeness comparable to manual compression. Despite the many advantages and variety of devices available, there are. A cardiologist would be best suited to treat patients with Coronary Artery Disease. Cardiologist are trained to help patients specifically with heart issues, such as diseases and or any condition which in this case would be CAD. A cardiologist would also be beneficial to a person with CAD because they can teach you more about risk factors and how you can improve your heart health.

There are many diagnostic tests that can be used to confirm if you have CAD, one major test would be a Cardiac Catheterization. A Cardiac Catheterization is a procedure that can detect many other heart diseases but in this case it is a major test that can be used to detect CAD. Then the catheter is threaded through the arterial system until it reaches the coronary heart artery. The catheter will be visible on an x-ray screen for the doctor or surgeon can see internally. The Doctor will then thread a very thin wire to the catheter across the blockage cartilage. Right above this wire, a catheter with an expandable balloon is passed to the blockage.

Once the balloon is inflated it will push the plaque to the side to open the artery and allow blood to flow through. In more than most, patients will receive a Stent which is a collapsed wire mesh tube , this procedure is similar to angioplasty except a stent is permanent and angioplasty is temporarily. This stent is mounted on a special balloon, and is moved over the wire to the blocked area. When the balloon is inflated it opens and locks the stent in the artery, keeping it open. Lastly the balloon and catheter are taken out, and the artery will remain open and your heart will receive the blood it …show more content… Both of these surgeries have the same concept in mind, which is to allow blood to flow through the heart, but has different ways of accomplishing it.

The coronary Artery Bypass surgery will surgically remove a piece of vein from either the leg, artery from the chest, or the wrist and attach it above or below the blockage to allow the heart to receive blood. Angioplasty is also a surgery, the doctor will numb your wrist or groin and insert a catheter with a thin balloon attached to it. This balloon will be placed through the blockage and inflated to push the cartilage aside and allow blood to flow through.

But what are the benefits of cardiac catheterization in stable CAD? An older trial, known as the COURAGE trial, found that in patients with stable CAD, stenting plus medication therapy did not reduce the risk of death, heart attack, or other major cardiovascular events compared to medication therapy alone. However, stenting did provide symptom relief much quicker than medication therapy alone. The researchers compared outcomes in patients who underwent cardiac catherization, along with angioplasty with stenting or CABG when feasible, plus medications, to patients who received medication therapy alone. The study found that there was no difference between the two groups in the primary endpoint a combination of death from cardiovascular causes, heart attack, cardiac resuscitation, or hospitalizations for unstable chest pain or heart failure.

The ISCHEMIA trial did find that there was a small increase in procedural heart attacks damage to the heart muscle caused by an interruption in blood flow to the heart during the procedure in patients who underwent cardiac catheterization. But there was an even greater increase in spontaneous heart attacks in patients who did not undergo cardiac catheterization. The study also found that patients who underwent cardiac catheterization had more symptom relief than medication therapy alone. As a result, treatment guidelines continue to recommend that all patients with stable CAD should first have their medications increased to maximally tolerated doses.

However, cardiac catheterization would be very appropriate if such patients continue to have unacceptable symptoms, have poor tolerance to their medication therapy, or both.
